I, the Legal Guardian of the registered child, hereby grant permission for Nute Ridge Bible Chapel to photograph my child during the VBC program for the sole purpose of the closing VBC program.
I understand that these photographs will be used exclusively for the closing program and will not be published on social media or used for any other purposes.

I, [Parent/Guardian Name], hereby release Nute Ridge Bible Chapel, its staff, volunteers, and affiliates from any and all liability arising from my child's participation in activities organized by Nute Ridge Bible Chapel.
I understand and acknowledge that my child's participation in activities at Nute Ridge Bible Chapel may involve certain risks, including but not limited to accidents, injuries, or illness. I hereby assume all risks associated with my child's participation, and I agree to hold Nute Ridge Bible Chapel harmless from any and all claims, damages, liabilities, or expenses that may arise as a result of my child's participation.
In the event of an emergency, I authorize Nute Ridge Bible Chapel and its staff to seek and consent to necessary medical treatment for my child. This includes, but is not limited to, first aid, administration of medication, and transportation to a medical facility. I understand that every effort will be made to contact the emergency contacts provided at registration prior to seeking medical treatment, if time allows.
If a life-threatening situation arises and immediate action is required to protect the health and safety of my child, I authorize Nute Ridge Bible Chapel and its staff to use emergency services, including but not limited to calling 911 or other emergency medical services. I understand that any costs incurred as a result of emergency services will be my responsibility as the parent/guardian.
